FILE UPLOAD INSTRUCTIONS:

Fill out this form with your contact details and track information, check the service agreement box, and press the submit button.
On the next page you will select your audio tracks from your computer and upload them to us through the web page.
After the upload is complete you will be taken to a payment page to pay for your services.

If you wish to mail in your project please see the instructions at the bottom of this page.

List your track details in the order you wish them to appear on the final Master CD

Track Position:	Audio File Name: Indicate the name of the file to be uploaded for use in each track position.	Track Title: Enter the track name as you would like it to appear on the final Master CD	Additional Notes: Such as different artist names, authors, fades, notes on sound, ISRC codes, inter-song spacing ( 2 to 3 seconds is standard ), any special editing needs or other requests, etc.

By checking this box I agree to the charges for services to be rendered by Crazy Daisy Productions as listed on my order form. Guarantee: You will receive a great-sounding, high-quality audio master. If you are unsatisfied with any portion of the master cd, we will redo that portion to make it sound right for you. It's our job to help you sound great! Cancellation policy: An order may be cancelled anytime prior to the start of your project at our studio. Once work has commenced on your project, your order may not be cancelled. Audio Sample Release (optional): You must be authorized to release the rights to the audio tracks listed in order to agree.
I hereby authorize Crazy Daisy Mastering to use up to 30 seconds from my specified audio tracks for Crazy Daisy Mastering's promotional purposes. I understand this is entirely optional and that if my sample is chosen to be shared Crazy Daisy Productions will include a link to the website I have listed on this order form next to my audio sample as compensation for use of my sample.

If you experience problems submitting the form, you are welcome to copy/paste the entire filled-out form page into the body of any email to us at info@crazymastering.com and submit your audio files via a web transfer service or your own ftp file upload system.
